Step 2

Step 3

Step 4

Completed Form DS-82 should be mailed along with the current U.S. passport, certified copy of court order or the marriage certificate, two recent passport photos and the applicable renewal fee. In the form of check or money order and make it payable to “U.S. Department of State” the renewal fee should be sent to the address listed on Form DS-82 using a traceable form of delivery.

Step 5

Only if the name change cannot be legally documented or if the name change is for a minor who is under the age of 16 the passport should be renewed in person. Step 6

Step 7

For standard processing of all mailed applications it takes about four to six weeks. Along with any applicable renewal fees you can expedite the processing of the renewal application by including a $60 expedition fee.
