The aim of incident report is documenting the apt details about the occurrence of an incident at the time of being them fresh in mind of the person who actually witnessed the incident.The information by the documentation prepared can be made useful for the future while dealing with any issues which may follow the incident.
An ideal document reporting an incident should have many necessary features so that they can be used effectively for further investigation that may follow. Then the incident is finally described and the impact of the incident is noted.

The problem usually terminates if the cause does. Thus the sensible elimination of the root cause leads to the termination of the problem occurred or likely to occur. For creating an incident report identification of a root cause or a series of root causes is identified through the investigation of the incident. Then a set of preventive or corrective actions are assigned to the single cause or the number of causes which were actually found.
The collection of data, conclusions and the recommendations are very important and also the actual intended audience is the equally most important matter of consideration. Some times for reporting certain incidences there may be a need for multiple reports.Whoever writes the reports of incidents must bear it in mind that various reports portray the actual incident in same manner and interpretation of data is done similarly with the final conclusions and further recommendations.
