Most high schools require that all students have a sports physical exam before they begin paying in their sport of choice. Not a substitute for a full physical examination, the sports exam is used to check for potential athletic problems and is comprised of two parts: a medical history and a physical exam.

The medical history section of the exam is typically directed to the parent as well as the athlete, and asks a comprehensive list of questions designed to pinpoint any potential health problems arising from the student’s past health as well as their family health history. The standard questions include any illnesses such as diabetes, asthma, epilepsy as well as any allergies, previous concussions, and excessive weight loss or gain. In addition, the questionnaire will ask whether the student’s immunizations are up to date and whether they have had a tetanus immunization. Female students will answer questions about their menstrual cycle and any abnormalities in their menstrual function history.

If any problems are found, the doctor will typically request that the student receive follow-up care with their regular health care provider for further examination, tests, or treatments before the student is allowed to practice or play in their sport. If everything checks out fine, the doctor will fill out a form stating the student is healthy to participate in sports.
