Botulinum toxin (Botox) was first purified in the 1970s by Dr. Edward Schantz at the University of Wisconsin–Madison. It was discovered that small amounts of botulinum toxin injected into muscles could paralyze them. In 1989, Ophthalmologist Dr. Alan B. Scott first used botulinum toxin to treat blepharospasm (uncontrolled blinking) and strabismus (misaligned eyes). The U.S. Food and Drug Administration (FDA) approved botulinum toxin cosmetic to treat frown lines between the brows in 2002.

Composition And Mechanism Of Action

Botulinum toxin is a neurotoxic protein produced by the bacterium Clostridium botulinum. There are eight different types of the toxin, termed serotypes A through H. Botox Commercial forms of botulinum toxin contain tiny amounts of purified botulinum toxin serotype A, the most common cause of botulism in humans.

Botulinum toxin works by blocking the release of a chemical called acetylcholine from nerve endings in the injected muscles. Acetylcholine is a neurotransmitter that allows muscle contractions. When botulinum toxin is injected into muscles, it binds strongly ...
... to the motor nerve endings and inhibits the release of acetylcholine at the neuromuscular junction for three to four months, resulting in reduced muscle contraction and relaxation. This temporarily weakens or paralyzes the muscles and reduces wrinkles or excessive sweating.

Uses Of Botox

Cosmetic Uses

The most common cosmetic use of botulinum toxin is to reduce frown lines, crow's feet and forehead wrinkles. By paralyzing the underlying muscles that cause these wrinkles to form, botulinum toxin can smooth the appearance of moderate to severe frown lines, crow's feet and forehead wrinkles for an average of four months. Many dermatologists and plastic surgeons also use it to enhance lip or chin contours. Some also get it injected into other areas like the masseter muscle in the jawline to slim a square face shape.

Medical Uses

Botulinum toxin is also widely used off-label to treat medical conditions like chronic migraines, excessive sweating, TMJ disorders and more. It can alleviate pain and reduce the frequency of migraines and tension headaches when injected into specific head and neck muscles. Excessive underarm sweating, known as hyperhidrosis, can also be effectively treated with botulinum toxin injections into the sweat glands.

Side Effects Of Botox

Common Side Effects

The most common potential side effects associated with botulinum toxin injections include:

- Bruising or swelling at the injection site

- Headache

- Weakness or muscle soreness at the injection site

- Flu-like symptoms such as fever, body aches and fatigue

Most side effects are mild to moderate in severity and last less than two weeks. Applying an ice pack, taking over-the-counter pain relief medication, and avoiding strenuous exercise for a day can help minimize discomfort at the injection site.

Rare Serious Side Effects

Rare but more serious potential complications include:

- Drooping of eyelids or eyebrows (eyelid ptosis)

- Asymmetrical facial appearance with uneven appearance of eyebrows, eyelids or smile

- Double or blurred vision due to weakening of eye muscles

- Spread of toxin effects to other areas of the body such as neck and appendages

- Allergic reaction to botulinum toxin or excipient ingredients in the formulation

Precautions With Botox

Botox injections should be avoided during pregnancy and breastfeeding due to a lack of safety studies. Patients with neuromuscular disorders like myasthenia gravis or motor neuron disease are also typically not considered candidates for treatment. Those with infections at injection sites should wait until the infection clears up before treatment.botulinum toxin can be safely administered for cosmetic and medical uses when handled appropriately by an experienced clinician.
