Hyperthermic Intraperitoneal Chemotherapy (HIPEC) is a concentrated & heated chemotherapy treatment delivered directly to the abdomen during surgery. While chemotherapy circulates throughout the body through the bloodstream, HIPEC allows delivery of chemotherapy exclusively to the cancer cells in the abdomen.
Prior to administering HIPEC treatment, cytoreductive surgery is performed to remove as many visible abdominal tumours as possible.
Cancers sometimes present with spread to the lining of the abdomen (peritoneum) called as peritoneal carcinomatosis. This kind of spread can occur from cancers of stomach, colorectal cancers, ovarian cancer, cancer of appendix, psuedomyxoma or mesothelioma. These advanced cancers are difficult to treat. Traditionally they are treated with chemotherapy with very low chance of cure. However, when the disease is confined to the abdomen, HIPEC becomes an effective option in selected candidates.
Once that process is completed, a heated chemotherapy solution is delivered to the abdomen to destroy the remaining cancer cells.

How is HIPEC Done?
HIPEC procedure consists of 4 stages:

o Resection

Exploration:
The procedure begins with an in-depth analysis of the abdominal cavity to determine the extent of peritoneal cancer with precision. Peritoneal Cancer Index (PCI) is the most common score used to quantify the extent of peritoneal cancer. PCI helps your surgeon to decide whether the visible cancer cells can be removed through surgical resection
Resection:
Cytoreductive Surgery involves surgical removal of surfaces and organs invaded by cancer cells in the peritoneal cavity. The number of resections differs from one patient to another. Even after surgical resection, it is highly likely that microscopic cancer cells still survive. If untreated, they multiply and lead to cancer recurrence.
HIPEC:
HIPEC is administered in the abdominal cavity to destroy the remaining invisible tumour cells. To increase the effectiveness of chemotherapy, it is heated to 41-42 degrees Celsius. Heating of the chemotherapy solution is done by a dedicated perfusion device. The procedure can last anywhere between 30 to 90 min. The chemotherapy solution is then drained from the abdomen and the incision is closed.
Reconstruction:
Once the procedure is complete, various catheters inserted in stomach, urinary bladder or abdominal cavity are removed depending on the condition of the patient post surgery.
Who can benefit from HIPEC?
HIPEC can be a life saver in most cases. However, not all patients derive positive results from this procedure. Following are some of the factors that need to be considered:
o Origin of Peritoneal Cancer
o Extent of the disease
o Patient’s clinical condition
HIPEC is a good treatment option for patients with advanced surface spread of cancer within the abdomen, but it is not very effective with cancer outside the abdomen.
What are the Likely Complications of HIPEC?
o Infection of the Lungs
o Infection of Urinary Bladder
o Inability to eat or drink for several days
o Wound problems
o Leakage of bowel contents into the abdomen resulting in abdominal infections
Despite the complexity of HIPEC procedure, research indicates that the Quality of Life is preserved after HIPEC and may even improve in some patients.
